NATIONAL TEAM REGISTRATION FORMS

PLEASE READ THE INSTRUCTIONS CAREFULLY BEFORE FILLING OUT THE FORM

• Kindly fill out the registration forms by entering the information into the grey entry fields provided.
• Page 2 is for registration of all NF Officials. The first 3 rows are only for registration of President, General Secretary and Team Manager. The remaining rows will allow you to register Coach, Referee, NF Official or VIP. Please select from the drop down list in the POSITION column.
• Pages 3-7 are for registration of Athletes in Male A, Male B, Female, Junior Male and Junior Female respectively. Each NF is permitted to enter only one boxer per weight division.
• All international spectators are to be registered separately on the Spectator Registration Form.
• This form is for members of your National Team only.
• All registration forms must be submitted together with ID photos in either JPEG or PDF format of all participants entered on the form. Please name each photo file as follows: PHOTO_COUNTRY_POSITION_FIRST NAME_SURNAME:

For example, the photo for a Thai athlete in Male A 67 KG should be saved with the name: PHOTO_THAILAND_ATHLETE_MALEA67KG_SOMCHAI_PANTIP.jpeg. This will help to make accreditation process smoother and faster.

• All registration forms must also be submitted together with all passport scans of all participants in either JPEG or PDF format entered on the form so that we can send the info to the relevant Thai Embassies to help to facilitate the visa issuance. Please name each passport copy file as follows: PHOTO_COUNTRY_POSITION_FIRST NAME_SURNAME:

For example, the passport copy for a Thai athlete should be saved with the name: PASSPORT_THAILAND_ATHLETE _SOMCHAI_PANTIP.jpeg.

• Deadline for sending all registration forms is 1^st July 2012. We DO NOT guarantee visa assistance/letter for registrations submitted after the closing date.
